In 2020, the Barangay Flores Health Center in Malabon City began offering SOAP (Subjective, Objective, Assessment, Plan) for patients’ medical check-ups, lab tests, and follow-up consultations at each appointment. The services provided reflect the essential roles of three healthcare providers: the Barangay Health Worker (Helper), the Doctor, and the Encoder.
